If that is the problem then here are some blank forms that I made up for you. The forms changed over the years and I made some of them but not all. Because I got tired of making blank forms. I am not the greatest form maker but hopefully this will be able to assist you somewhat.   Most of the clerks used the form with the preprinted date in the 1920s, like 192__. The early years asked the least amount of information and the later years the most. Not every year asked for the spouse's name. Basically that was only on the forms used in the 1930s and 1940s.
